Please note that benefits are different between each plan, so even if I am in network, coverage is not guaranteed
If I am not in network with your insurance but you have out-of-network benefits, I can provide you with a superbill that you may submit to your insurance company for potential reimbursement.
Self-pay rates: $150/55 min session, $75/30 min session
*Self-pay will not go be billed through insurance therefore will not contribute towards deductible

Late cancel/no show policy: No shows or cancellations within 24 hours will incur charge of full session cost, at the provider's discretion (ex: excluded in the case of emergency)

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including nutrition services. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
