Insurance accepted: United Healthcare, Regence/Blue Cross, Cigna, Pacific Source, Moda

Please note that benefits are different between each plan, so even if I am in network, coverage is not guaranteed

 

If I am not in network with your insurance but you have out-of-network benefits, I can provide you with a superbill that you may submit to your insurance company for potential reimbursement.

 

Self-pay rates: $150/55 min session, $75/30 min session

*Self-pay will not go be billed through insurance therefore will not contribute towards deductible

To verify your nutrition benefits through your insurance call the number on your insurance card and ask:

  • Is Samara Roman-Holba an in-network provider under your plan, or do you have out of network benefits?

  • If you have nutrition counseling benefits for CPT codes 97802 and 97803 and diagnosis code z71.3 or an eating disorder diagnosis?

  • Do you need to meet your deductible before visits are covered?

  • Is there a copay/coinsurance?

  • Is there is a limit on the number of visits allowed per year?

  • Ask for a reference number for the call as well

Late cancel/no show policy: No shows or cancellations within 24 hours will incur charge of full session cost, at the provider's discretion (ex: excluded in the case of emergency)

*Good Faith Estimate: Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including nutrition services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including nutrition services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
